New Patient Special - First Visit for $49!

If you would like to see Dr. Daulton:

• Call (219) 932-8900 to make an appointment, or scroll down to make an appointment online.
• Dr. Daulton will do a complete and thorough examination.
• Any required X-rays will be taken.
• A chiropractic adjustment will be provided on the first visit.
• He will review all your findings with you.
• He will then design a treatment plan to get you well!
• All fees will be discussed before any care is given. We will never surprise you with a bill.

Paying for your care:

• Auto Accident injuries are covered under the Medical Pay coverage available on your personal auto insurance. We will need to verify the information first.
• Work Comp injuries are covered 100% in Indiana, however, your employer has the right to choose your doctor. Approval will need to be obtained by your employer and workers compensation insurance company first.
• Medicare does not pay for exams and x-rays so the first visit will be $49. Medicare then covers 80% up to 4 weeks.
• Regular health insurance - We are not in-network and do not accept any as they rarely pay anything anyway. The first visit fee is $49.
• No insurance? Great, the first visit fee is $49 then we will create an affordable plan to get you feeling great again!
